Health ID Card Online Application हेल्थ आईडी कार्ड ऑनलाइन आवेदन  

कोरोना महामारी के कहर को देखते हुए प्रधानमंत्री मोदी द्वारा 74 वें स्वतंत्रता दिवस के अवसर पर स्वास्थ्य सेवाओं की पहुँच प्रत्येक नागरिक तक सुलभ बनाने के लिए डिज़िटल हेल्थ मिशन योजना की शुरुआत की गयी है। इस मिशन के अंतर्गत नागरिकों के स्वास्थ्य सम्बन्धी डाटा को ऑनलाइन सुरक्षित किये जाने की योजना बनायी गयी है। नागरिकों के स्वास्थ्य रिपोर्ट की हिस्ट्री, सभी सरकारी स्वास्थ्य संस्थान और सरकारी स्वास्थ्य केंद्र के डॉक्टर्स को नेशनल हेल्थ अथॉरिटी (NHA) पोर्टल से जोड़ने के लिए उनके डेटा को पोर्टल पर सुरक्षित किया जाएगा। इस पोर्टल से जुड़े सभी चिकित्सा केंद्र और चिकित्सक की विशेषताओं की जानकारी ऑनलाइन प्राप्त की जा सकेगी।

नेशनल डिज़िटल हेल्थ पोर्टल पर रजिस्टर्ड नागरिकों को एक यूनिक हेल्थ आईडी प्रदान की जायेगी। हेल्थ आईडी कार्ड धारक व्यक्ति को देश में कहीं भी चिकित्सा सुविधा का लाभ लेने के लिए मेडिकल रिपोर्ट की हार्ड कॉपी लेकर जाने की आवश्यकता नहीं होगी। पोर्टल पर रजिस्टर्ड स्वास्थ्य संस्थान, डॉक्टर्स और दवा की दुकान से स्वास्थ्य सुविधा का लाभ प्राप्त करने के लिए केवल हेल्थ आईडी नंबर बताना हीं काफी होगा। हेल्थ आईडी नंबर को नेशनल हेल्थ अथॉरिटी पोर्टल से जुड़े सॉफ्टवेयर में दर्ज करने पर रोगी की मेडिकल हिस्ट्री देखी जा सकेगी।

हेल्थ कार्ड धारको को  घर बैठे डॉक्टर से परामर्श प्राप्त करने के लिए बार -बार अपनी मेडिकल रिपोर्ट को ऑनलाइन अपलोड नहीं करना पड़ेगा। इसके अतिरिक्त देश भर में कहीं भी जाकर इलाज करवाने के लिए मेडिकल रिपोर्ट साथ लेकर जाने की आवश्यकता नहीं होगी न हीं दवा की दुकान पर दावा खरीदने के लिए डॉक्टर की पर्ची लेकर जाने की आवश्यकता होगी।

इन सभी सुविधाओं का लाभ प्राप्त करने के लिए आपके पास डिज़िटल हेल्थ आईडी कार्ड होना आवश्यक होगा। डिज़िटल हेल्थ आईडी कार्ड बनवाने के लिए ऑनलाइन आवेदन एनएचए  ( National Health Authority) पोर्टल के माध्यम से किया जा सकता है। हालाँकि अभी सभी प्रदेशों के नागरिकों के लिए डिज़िटल हेल्थ आईडी कार्ड आवेदन की सुविधा शुरू नहीं हो सकी है। फिरहाल देश के छह राज्यों में शुरू किया गया है। जल्द हीं सभी राज्यों के नागरिक ऑनलाइन डिज़िटल हेल्थ आईडी कार्ड आवेदन का लाभ उठा सकेंगे। आइये जाने डिज़िटल हेल्थ आईडी नंबर प्राप्त करने के लिए ऑनलाइन आवेदन की जानकारी।

डिज़िटल हेल्थ आईडी कार्ड बनवाना सभी नागरिकों की मर्जी पर है। इसी तरह निजी चिकित्सा केंद्र और उनमें कार्यरत डॉक्टर भी अपनी स्वेच्छा से इस योजना से जुड़ सकते है। वर्तमान में हेल्थ आईडी कार्ड ऑनलाइन आवेदन की सुविधा निम्नलिखित छह राज्यों में हुई है :

  • अंडमान एंड निकोबार आइलैंड
  • लक्षद्वीप
  • लद्दाख
  • पुदुचेर्री
  • दादरा एंड नगर हवेली एंड दमन दिउ
  • चंडीगढ़

Health ID Card Online Application Documents  हेल्थ आईडी कार्ड ऑनलाइन आवेदन दस्तावेज़

  •  आधार कार्ड नंबर
  • मोबाइल नंबर
  • जन्म तिथि

Health ID Card Online Application Process  हेल्थ आईडी कार्ड ऑनलाइन आवेदन प्रोसेस 

  • डिज़िटल हेल्थ आईडी कार्ड ऑनलाइन आवेदन करने के लिए लिंक पर क्लिक करिए।
  • पोर्टल पर डिज़िटल सिस्टम्स विकल्प के अंतर्गत create health ID विकल्प पर क्लिक करना है।
  • इस पेज में continue विकल्प पर क्लिक करना होगा।
  • फिर genertae via Aadhaar विकल्प पर क्लिक करना होगा। यदि आधार कार्ड नंबर के उपयोग से आवेदन नहीं करना चाहते हैं, तो मोबाइल नंबर के माध्यम से आवेदन के लिए click here विकल्प पर क्लिक करना होगा।
  • इसके बाद दूसरे पेज में आधार कार्ड नंबर /मोबाइल नंबर लिखना होगा। फिर आपके द्वारा दर्ज किये गए मोबाइल नंबर/ आधार कार्ड नंबर से लिंक्ड मोबाइल नंबर पर OTP का मेसेज प्राप्त होगा। OTP लिखने के बाद verify विकल्प पर क्लिक करना होगा।
  • इसके बाद व्यक्तिगत जानकारी फॉर्म में दर्ज करके एनएचए पोर्टल पर अपना प्रोफाइल बनाकर पोर्टल पर रजिस्टर करना होगा।
  • इसके बाद डिज़िटल हेल्थ आईडी कार्ड के लिए आवेदन फॉर्म भरने की प्रक्रिया पूरी होने पर submit विकल्प पर क्लिक करना होगा।
  • अब आपका हेल्थ कार्ड जेनेरेट हो जाएगा। आप इस कार्ड को डाउनलोड कर सकेंगे।
